Q:  Could my dog have an ear infection if he is always shaking his head?

A:  While I cannot make a diagnosis without examining your dog myself, I find in most cases head shaking does mean an ear infection or that something has made its way into a dog's ear. I would take your dog to the vet to get his ear looked at.
